The Tipton County Adult Day/Respite Service,  also known as “The Club”, is a social model day program. 

 

"The Club" - Covington is located inside the Covington Senior Center at 401 S. College Street
(at Church Street)

 
Hours: Monday, Wednesday, Friday 9 a.m.  - 3 p.m.

 

"The Club" - St. Luke at St. Luke Baptist Church off of Hwy 14 N.

 

Hours:  Tuesday & Thursday 9 a.m. - 3 p.m.

 

 

 

Program Cost:     $30.00 per day

 

    Matching Scholarships are available to qualifying participants

    Medicaid Waiver is also available.  Please call for details

‘The Club” is funded by Tipton County Government, the City of Covington, the Brookdale Foundation and private donations.

 

The purpose of the Tipton County Adult Day/Respite Service is threefold:

·         to provide significant periods of respite for family caregivers,

·         to provide a safe stimulating environment for those with dementia from Alzheimer's Disease, Parkinson's disease, stroke effects or the like, and

·          to delay institutionalization (nursing home) for the care recipient

How to make a referral or enroll …

 

   

 

· Referrals are accepted from all sources - Typically the family caregiver makes

    the phone call 901-476-3333

 

· Referrals may be made 8am to 2pm, Monday thru Friday – A pre-enrollment

     packet will be provided upon request

 

· Certification by a physician of a diagnosis of dementia along

with a functional assessment of the applicant is required

 

· A functional assessment is performed during the pre-enrollment visit 

of applicant and primary caregiver at the ADRS office at TCCA

 

· To make a referral call the Tipton County Commission on Aging       

and ask for Adult Day Respite Staff
